Learn how to take control of your chronic condition – and live healthy.
If you’re one of the millions of Americans living with a chronic condition, Aetna Health ConnectionsSM may help. It’s an entirely voluntary and confidential program that takes a new approach to chronic disease – one that looks at your entire health picture, not just your condition. The program provides support for more than 34 conditions in categories that include:

  • Vascular (blood, veins, arteries)
  • Pulmonary (lungs)
  • Gastrointestinal (stomach and intestines)
  • Neurological (nerves)
  • Orthopedic (bones and muscles)
  • Oncology (cancer)

. . . and others, such as cystic fibrosis, chronic kidney disease, low back pain and sickle cell disease. The program even covers such issues as behavioral health and end-of-life care.

If you or a covered family member is eligible for Aetna Health Connections, a program nurse may contact you.
Or you can get started on your own. Just log on to Aetna Navigator and look under the Benefits tab for “Health Programs.” Under “Condition Management” you’ll see “Enroll in Aetna Health ConnectionsSM Disease Management.” Complete the survey and the request for information.
