Using the
Aetna EPN Plan
You don’t have to choose a PCP, but you might want to. Your PCP helps you learn about your health and how to manage it.
- You have direct access to Specialists without a PCP referral
- When you visit a physician, you will pay the applicable PCP/Specialist copay
- No claim forms or balance bill
- Coinsurance applies for hospital-based services
- Except for emergency or out-of-area urgent care, benefits are not covered outside of the network
- Wellness and preventive programs are included
Transitioning Your Care
If you are in an active course of treatment on the day that your EPN coverage first takes effect,
and your provider is not part of Aetna’s network, call Aetna Member Services at 1-888-744-2537 to discuss your care options, or you and your provider can complete the Transition of Care Form [PDF] and submit it to Aetna for review.
After your doctor fills out Section 4, fax the completed form to 1-860-907-3739 to the attention of the Verizon CAT Team Leader (per instructions provided on the form).

- Select Geographic Information
(look up by Zip Code, City, or County)
- Select the Provider Category
(medical, dental, hospitals, etc.)
- Select the Provider Type
(Primary Care, Specialist, etc.)
- Select the Plan Type - “EPN”
If desired, you may check the “Additional Search Criteria” box to narrow the search by specialty, name, hospital affiliation, etc.
- Click on "Start Search"
